Scientists at Florida International University have identified physical changes in the brains of people with common sleep disorders. The findings, published in Scientific Reports, suggest that conditions like insomnia and sleep apnea are associated with reduced volume in specific brain regions. These areas are critical for attention, motivation, and decision-making.

The study combined data from 57 previous studies involving brain imaging of adults. This large-scale analysis allowed researchers to compare different types of sleep disturbances and identify both shared and unique structural patterns. The results indicate that disrupted sleep is connected to important differences in brain architecture, potentially explaining why people with these disorders often struggle with focus and cognitive control.

    Thalamus Reductions Across Sleep Types

The research categorized sleep disorders into two main groups: dyssomnias and parasomnias. Dyssomnias include insomnia and sleep apnea, which interfere with the ability to fall or stay asleep. Parasomnias involve unusual behaviors during sleep, such as sleepwalking or night terrors.

Both groups showed a consistent reduction in the thalamus, a deep brain structure that processes sensory information and regulates consciousness. Specifically, the pulvinar region of the thalamus was affected. This area helps the brain direct attention to important stimuli and manage cognitive tasks. The researchers noted that damage or reduction in this region may explain the slower reaction times and increased error rates often seen in people with sleep problems.

Matthew Sutherland, a cognitive neuroscientist at FIU and senior author of the study, emphasized the importance of understanding these physical changes. He stated that bringing together results from many studies provides a clearer picture of how sleep disorders affect brain structure and function.

    Distinct Patterns in Parasomnias

While thalamus reduction was common to both groups, the analysis revealed distinct changes in people with parasomnias. Individuals with conditions like sleepwalking and nightmare disorder showed alterations in the posterior cingulate cortex. This region is involved in motivation, emotional regulation, and decision-making.

These findings may help explain why parasomnias are sometimes accompanied by difficulties with mood and behavior. In contrast, the study did not find a consistent pattern of structural changes exclusively linked to dyssomnias like insomnia and sleep apnea. This suggests that different forms of disrupted sleep may impact the brain in varied ways, depending on the specific nature of the disorder.

Katharine Crooks, the lead author and a recent FIU doctoral graduate, noted that most research looks at sleep disorders one at a time. By looking across many studies, her team found shared patterns that help explain why sleep problems can impact everyday functioning. She added that understanding these brain changes is key to finding better interventions and personalized treatments.

The study highlights a major public health issue, as an estimated 50 million to 70 million Americans experience sleep disorders. The findings could eventually help scientists identify sleep problems earlier and develop treatments tailored to the specific brain systems involved. Future research will focus on examining individual parasomnias in greater detail to further understand the relationship between sleep and brain health.
