Pneumonic Plague: What Is It and Why Is It in the News?

A 28-year-old researcher died last week in a Siberian laboratory, prompting an investigation into the cause of her death. While Russia’s consumer health watchdog stated that no pathogens linked to her work were found in her body, local media have raised concerns about the spread of pneumonic plague. The World Health Organization has noted that the full picture of the event is not yet clear, leaving experts and the public with questions about the disease.

    Understanding the pneumonic plague

The plague is caused by bacteria that can affect different parts of the body, leading to several distinct forms of the illness. The most common version is bubonic plague, which typically occurs when a flea bites a person. This form is characterized by swollen and painful lymph nodes, known as buboes. The term “bubonic” derives from the Greek word for groin, as these swollen nodes often appear in that area. Diagnosis usually involves testing blood or lymph node samples in a laboratory.

Pneumonic plague is a more severe variant that occurs when the bacteria spread to the lungs. According to the Centers for Disease Control and Prevention, this form often lacks the telltale signs of bubonic plague, making it harder to identify immediately. Medical experts describe it as significantly more dangerous because it can transmit easily between people through respiratory droplets. This airborne transmission means it can spread rapidly in close-contact settings.

The disease progresses quickly, and without prompt medical intervention, it can be fatal. Adelaide Miarinjara, a medical entomologist at Emory University, emphasized that early diagnosis is critical. She noted that if people do not seek treatment or are not expecting the disease, the risk of death increases significantly. Treatment typically involves antibiotics, often administered through intravenous infusion to ensure rapid absorption and effectiveness.

    Historical outbreaks and quarantine origins

Recent reports indicate that 200 contacts of the deceased researcher are in quarantine. This practice of isolating individuals to prevent disease spread has deep historical roots. The concept dates back thousands of years, with early references found in religious texts describing the isolation of those with leprosy. The modern term “quarantine” originated during the Black Death era in the city-state of Dubrovnik, now part of Croatia.

Dubrovnik leaders implemented a 40-day isolation period for visitors arriving by sea to protect the wealthy merchant city from the bubonic plague. The word comes from the Italian term for “40,” reflecting the duration of the wait on a remote island before entering the city. Breaking these isolation rules carried severe penalties, including physical mutilation or torture, underscoring the seriousness with which historical societies viewed disease control.

Modern outbreaks of pneumonic plague remain rare but can have significant impacts. In 2017, Madagascar experienced a large outbreak after the disease was brought from a rural area to the coastal city of Toamasina. The city had not seen a case in nearly a century. The outbreak resulted in more than 2,400 suspected cases, with nearly 1,900 being pneumonic plague. Studies from that time estimated a 25% mortality rate among confirmed cases, highlighting the ongoing threat of the disease in areas with limited healthcare infrastructure.

As investigations continue in Siberia, health authorities are monitoring the situation closely. The World Health Organization continues to assess the risk, noting that current information is incomplete. Public health measures, including contact tracing and isolation, remain standard protocols for managing potential outbreaks of infectious diseases like the plague.
