The U.S. Food and Drug Administration approved the first generic version of Climara Pro on Tuesday, marking a step toward broader access to hormone therapy for women. The generic estradiol and levonorgestrel transdermal system is therapeutically equivalent to the branded product and is applied once a week.
Zydus Pharmaceuticals (USA) Inc. received the approval for the generic patch. The medication delivers 0.045 mg of estradiol and 0.015 mg of levonorgestrel per day. It is indicated for use in women with an intact uterus to treat moderate to severe vasomotor symptoms associated with menopause.
Weekly Dosing and Uterus Protection
The generic patch contains both estrogen and progestogen in a single application. This combination distinguishes it from estrogen-only patches. The progestogen component is included to reduce the risk of endometrial hyperplasia, a condition that can develop when estrogen is used alone in women who still have their uterus.
Michael Davis, M.D., Ph.D., Director of the FDA’s Center for Drug Evaluation and Research, noted that the once-weekly dosing schedule offers a practical alternative to daily oral medications. He stated that a patch provides a convenient option that may help patients maintain their treatment regimen.
Acting FDA Commissioner Kyle Diamantas, J.D., emphasized the importance of affordable treatment options for a universal experience. “Menopause is a universal experience for women, and it can greatly affect a person’s quality of life. This approval makes effective treatment more accessible,” Diamantas said. He added that such approvals reflect the agency’s commitment to expanding patient access to safe and high-quality medications.
Safety Warnings and Adverse Reactions
The FDA has outlined specific contraindications for the generic estradiol and levonorgestrel transdermal system. Patients with undiagnosed abnormal genital bleeding, breast cancer, or a history of breast cancer should not use the patch. It is also contraindicated in individuals with estrogen-dependent neoplasia, active deep vein thrombosis, pulmonary embolism, or active arterial thromboembolic disease.
Other conditions that prohibit use include known hypersensitivity to estradiol or levonorgestrel, hepatic impairment, and protein C, protein S, or antithrombin deficiency. The most common adverse reactions, occurring in 5% or more of patients, include application site reactions, vaginal bleeding, breast pain, upper respiratory infection, back pain, depression, pain, headache, and flu syndrome.
Climara Pro is a registered trademark of Bayer. The approval aligns with the FDA’s Public Health Pillar initiative to increase access to affordable medicines. In the United States, nine out of ten prescriptions filled are for generic drugs, a trend that fosters market competition and lowers costs for patients.
Manufacturers are responsible for determining the availability of the generic patch in the market. Patients and healthcare providers should consult the full prescribing information for detailed safety data and usage instructions.
