Millions of Americans spend over $1 billion annually on supplements containing omega-3 fatty acids, hoping to preserve memory and protect their brains as they age. These nutrients are essential for building and maintaining the connections between brain cells that support learning and cognitive function. However, a new two-year clinical trial suggests that taking these supplements in pill form does not deliver the expected protection.

Researchers at Keck Medicine of USC published their findings in eBioMedicine, reporting that high doses of omega-3s failed to produce measurable improvements in memory, thinking abilities, or protection against brain shrinkage. The study involved older adults considered to be at increased risk of developing Alzheimer’s disease, yet those taking the supplements showed no significant cognitive advantage over those who received a placebo.

Study Results Show No Cognitive Advantage

The trial enrolled 365 adults between the ages of 55 and 80. All participants rarely ate fish, which is a primary dietary source of omega-3s, and nearly half carried the APOE4 gene variant, the strongest known genetic risk factor for late-onset Alzheimer’s. Participants were randomly assigned to receive either a daily supplement providing 2,000 mg of docosahexaenoic acid (DHA) or an inactive placebo.

DHA is a major component of brain cell membranes and supports communication between nerve cells. Before assessing cognitive outcomes, researchers needed to confirm that the supplements were actually reaching the brain. They measured DHA concentrations in cerebrospinal fluid, the liquid surrounding the brain and spinal cord. After six months, DHA levels in the cerebrospinal fluid had increased by an average of 17% among those taking the supplements.

Despite confirming that the omega-3 fatty acids successfully entered the central nervous system, the higher levels did not translate into better brain health. Researchers assessed memory and cognitive abilities at the start of the trial and again after two years. Those taking fish oil performed no better on cognitive tests than the placebo group. The supplements also failed to prevent shrinkage of the hippocampus, a brain region critical for forming memories that is particularly vulnerable to aging and Alzheimer’s disease.

Broader Dietary Patterns May Offer Better Protection

The unexpected findings have led researchers to question why higher levels of omega-3s in the brain do not necessarily improve its function. Hussein Naji Yassine, MD, director of the USC Center for Personalized Brain Health and lead investigator, stated that while omega-3s are important for brain cell connections, the results do not support fish oil supplements as a preventive measure against Alzheimer’s.

Yassine and his colleagues are now investigating how the brain absorbs, processes, and uses these nutrients. Their earlier research suggests that omega-3s may be more beneficial when consumed as part of a broader dietary pattern rather than taken in isolation. The Mediterranean-style diet, which emphasizes fish, vegetables, fruits, whole grains, legumes, nuts, and olive oil, naturally supplies omega-3s alongside other nutrients and has been associated with a lower risk of Alzheimer’s disease.

The researchers suspect that the combination of nutrients in a balanced diet, along with an individual’s overall health, influences how effectively omega-3s support the brain. Factors such as poor health, genetic risk, and age may change the brain’s ability to absorb and use these nutrients. The team is working to develop medications that may help the brain better utilize these essential fats to preserve cognitive function.

The findings do not mean that omega-3 fatty acids are unnecessary or that eating fish offers no health benefits. Instead, they indicate that taking high-dose DHA supplements alone did not provide measurable protection in the population studied. The researchers emphasize that protecting brain health requires a broader approach, including regular physical activity, sufficient high-quality sleep, and a nutritious, balanced diet. Yassine compared maintaining a healthy lifestyle to regular car maintenance, noting that the brain is more likely to lose function if health issues in other parts of the body go unaddressed.
