    Cochrane Review: E-Cigarettes Outperform Other Quit-Smoking Methods

    A major updated review by Cochrane has found that nicotine e-cigarettes help more people stop smoking than several standard approaches, including nicotine gum, patches, and behavioral counseling. The analysis, which included nearly 30,000 participants, provides high-certainty evidence that regulated nicotine e-cigarettes are a more effective tool for smoking cessation than traditional nicotine replacement therapies.

    The findings come from a living systematic review, a format that allows researchers to continuously update their analysis as new clinical trials become available. The latest version incorporated 80 randomized controlled trials involving 29,861 participants. Nine new trials were added to this specific update, reinforcing the consistency of the results.

    Higher Quit Rates Than Nicotine Replacement Therapy

    Researchers compared the effectiveness of nicotine e-cigarettes against several other methods. They found that e-cigarettes produced higher quit rates than nicotine replacement therapy (NRT), which includes products like patches and gum. Specifically, for every 100 people using e-cigarettes, approximately four additional individuals successfully quit smoking compared to those using NRT.

    The data also showed that nicotine e-cigarettes performed better than non-nicotine cigarettes, resulting in about two additional quitters per 100 people. When compared to groups receiving no support or only behavioral support, such as individual counseling or group programs, the e-cigarette groups also demonstrated higher quit rates. However, the researchers noted they had less confidence in this specific comparison due to potential bias in how some of the studies were designed.

    “We have very strong evidence that regulated nicotine e-cigarettes are substantially less harmful than smoking,” said Dr. Jamie Hartmann-Boyce, senior author of the review from the University of Massachusetts Amherst. “Based on the consistency of the findings here, it’s clear that they are a proven and viable option for people wishing to quit traditional cigarettes.”

    Safety Concerns and Regulatory Warnings

    While the review found no evidence that using e-cigarettes to quit smoking increased the risk of serious harm, the authors emphasized that longer-term research is still needed. The conclusions apply strictly to regulated, nicotine-containing e-cigarettes. The researchers warned that illicit products and devices containing other substances, such as THC, may present different and potentially higher risks.

    Dr. Hartmann-Boyce highlighted a critical distinction between legal and illegal products. “It’s worth highlighting that our review looks at regulated e-cigarettes containing nicotine, but many of those available on the market do not meet these standards,” he said. “Unlicensed e-cigarettes are potentially dangerous, with issues including unstable batteries and harmful chemicals.”

    He advised that anyone looking to use e-cigarettes to quit smoking should only purchase devices that have been tested and approved by their country’s regulatory body. In the United States, the FDA maintains lists of regulated devices, while the MHRA provides similar guidance in the UK.

    Major health organizations, including the World Health Organization (WHO), continue to issue warnings about e-cigarettes. The WHO notes that smoking remains the world’s leading cause of preventable disease and death, responsible for more than seven million deaths annually. Health officials have also raised concerns that e-cigarette use among children and young people can lead to nicotine addiction and subsequent use of tobacco products.

    Not a One-Size-Fits-All Solution

    The researchers stressed that e-cigarettes may not be the best quitting method for everyone. People who want to stop smoking are encouraged to speak with stop-smoking support services or their clinicians to determine the most appropriate approach for their individual needs. Vaping still carries health risks, but the authors state those risks are much smaller than the dangers associated with continuing to smoke cigarettes.

    Dr. Nicola Lindson, lead author of the review based at the University of Oxford, noted that quitting smoking is difficult and many people have tried multiple times without success. “For that reason, it is important that there are a number of different options available to help somebody quit, which could encourage them to keep trying,” Lindson said. “Although nicotine vapes are unlikely to be completely risk-free, they are less harmful than smoking, and that is the most important comparison to make for people who are still smoking tobacco.”

    Dr. Hartmann-Boyce views switching to e-cigarettes as a possible stage in the process of moving away from smoking, rather than necessarily the final destination. “For somebody who is already addicted to nicotine through cigarettes, having a safer alternative like an e-cigarette is something worth considering, especially when other quit aids have proven unsuccessful,” he said. The research team is currently working on a review of interventions to help people quit vaping, which could be the next step for those looking to transition to a lower-harm product before quitting nicotine altogether.
